- Bring all current medication
- Bring your insurance card
- New patients arrive 20 minutes prior to appointment time
- New patients should print, complete and bring all New Patient Forms to your appointment
- To allow all open appointments to be filled for those who are sick, a no show fee of $25 may be assessed if you do not arrive for your scheduled appointment
